If you’re completing a SafeGard intake for a traveler under 18 years old, the process depends on whether the child is traveling alone or with an adult who is also completing a SafeGard intake.
If your child is traveling alone
A parent or legal guardian must verify their own identity before completing the child’s form. This ensures compliance with telemedicine and identity-verification requirements.
Step 1: Parent/Guardian identity verification
Create your own SafeGard account using your email address.
When prompted for identification, upload your government-issued ID or passport — not your child’s.
Once your identity is verified, you can begin a new intake for your minor traveler.
Step 2: Create a new travel intake for your child
Go to your home screen.
Click “+ Create new travel intake.”
Select “This form is for someone else.”
Enter your child’s personal information, health history, and immunization records.
If the traveler is under 18, SafeGard will not require a separate identity verification for them.
If your child is traveling with you (and you are also a SafeGard patient)
If both you and your child are traveling, you can add your child as a family member from within your own account.
Follow the steps outlined in How do I add family members or multiple travelers to my SafeGard intake?
You’ll complete your own identity verification and intake first, then create your child’s intake as an additional traveler.
Your own identity verification will cover both you and your child.
Important notes
Only the parent or legal guardian needs to provide a photo ID and complete the identity verification process.
Each child traveler needs their own travel intake form and voucher.
Use the parent or guardian's email address for login and communication — all updates and recommendations will be sent there.
Once completed, your child’s intake will follow the same physician review and pharmacy process as any other SafeGard traveler.
