You can edit your form directly.
- Log in to your SafeGard account at 🔗 https://patient.sgtravelmed.com 
- Open your travel intake from the home screen. 
- Update your destinations, dates, or other details as needed. 
- Save and continue when finished. 
If you’ve already submitted your intake
Once your intake has been submitted, it’s locked for physician review and cannot be edited.
- Contact SafeGard support to request an update. 
- Our team can reopen your form or assist with revising your recommendations if your travel plans have changed significantly.
